* This is the first year that Sport Physicals by the School Doctor will be held in June.
The dates of the Fall Sport Physicals are as follows:
Tuesday June 3, 2008 @ 3 P.M. - H.S. Health Office
Tuesday August 19, 2008 @ 8 A.M. - H.S. Health Office [M.S. students may also attend if needed]
* THE QUESTIONNAIRE MUST BE COMPLETED & SIGNED BY THE PARENT PRIOR TO THE SPORT PHYSICAL EXAM BY THE SCHOOL DOCTOR
IT IS ALWAYS BEST TO HAVE PHYSICAL EXAMS DONE BY YOUR OWN PRIVATE DOCTOR.
- ALL PHYSICAL EXAM DATES MUST BE LESS THAN ONE YEAR FROM THE START OF THE SPORT SEASON THAT THE STUDENT WILL PARTICIPATE IN.
* ALL SPORT PHYSICALS MUST BE ON THE HEWLETT- WOODMERE
SPORTS PHYSICAL EXAMINATION FORM
FOOTBALL - All Sport Physical Forms & Update Forms must be handed in by Thursday 8/14/08 to be on the roster for the morning of Monday, 8/18/08.
ALL OTHER SPORTS - All Sport Physical Forms & Update Forms must be handed in by Thursday 8/22/08 to be on the roster for the morning of Monday. 8/25/08.
