Welcome back to a new school year. As always, I enjoy my job as the district Transition Coordinator and I am ready to embark on another wonderful year working with students, parents, and staff.
My job as the Transition Coordinator is to facilitate the growth of the student collaboratively with school district staff and parents to ensure successful transition from school to the adult community, including work, future education, living, and participating in the community.
The transition process is designed to help your son or daughter move smoothly to adult life. This process formerly begins when your child reaches age 14. However, at age 12 a Level I Vocational Assessment which is made up of a student, parent, and teacher questionnaire is completed. Transition planning is part of the Individualized Education Plan (IEP) and will be reviewed and updated each year. The transition process focuses on the student’s interest and abilities, and to develop a plan for the future.
I will be visiting the classrooms to work with the students on the Level I Assessments and Transition Assessments during the month of October and November. The parent questionnaire will be mailed home to you in the coming weeks to be completed and returned.
